Supporting older adults and people with disabilities in weighing person-centered choices of available resources to discover their values and preferences. Providing follow-up to assess if the supports are working and if changes are needed. We offer a person-centered approach, addressing the individual’s needs and preferences.

Type of Services Provided

The ADRC offers a person-centered approach, working to address the individual’s needs and preferences. Afterwards, follow-up is also provided to assess if the supports are working or if additional services are needed.
  • checkedExploring the individual’s personal history, including preferred care setting and goals for the future, abilities, financial situation, and other information to help identify and evaluate available options.
  • checkedLearning about opportunities and resources for maximizing independence, including the support from family, friends, and community;
  • checkedInformation about no cost and private pay long term support and service options.
  • checkedDetermining long-term care options available to the individual, which may include but is not limited to:
  • Case management services
  • In home care
  • Caregiver support
  • Use of assistive technology
  • Community programs
  • Assisting living

How Options Counseling Works

  • checkedOptions Counseling will be provided at a time and place convenient for the individual. Options Counseling can include the individual and any other family members or support persons they choose, and can be done face-to-face (including in an ADRC-SBC partner’s office, or in the individual’s home or temporary care setting), over the internet (i.e. Zoom), by phone, email or mail;
  • checkedOptions Counseling is unbiased, timely, accurate, thorough, and appropriate to the individual’s current situation.
